To find Medicare providers using the Medicare Lookup Tool, start by visiting Medicare.gov and clicking on “Find care” in the main navigation menu. This will take you to the provider search tool where you can search for doctors, hospitals, nursing homes, home health services, hospice care, rehabilitation facilities, long-term care hospitals, and dialysis facilities.
Begin your search by entering your ZIP code or city and state in the location field. Then select the type of provider you need from the dropdown menu. You can refine your search by distance, with options ranging from 1 to 100 miles from your location. For doctors and clinicians, you can further filter by specialty, gender, and whether they accept Medicare assignment.
Once you click search, the results will display providers nearest to you with key information including addresses, phone numbers, and quality ratings where applicable. For hospitals and facilities, you’ll see star ratings based on patient experience and quality measures. Click on any provider’s name to view detailed information including services offered, accepted insurance, and for facilities, their inspection history and staffing levels.
