– Your body mass index (BMI) is at least 25 (or at least 23 if you are of Asian descent).
– You have not been previously diagnosed with diabetes, whether type 1 or type 2.
– You do not have a diagnosis of End-Stage Renal Disease (ESRD).
– You have not taken part in the Medicare Diabetes Prevention Program before.
– There is no cost for this service if you qualify.

Certified Medicare Diabetes Prevention Program providers deliver these services. The providers can range from established medical practitioners to entities such as neighborhood organizations or religious institutions.
